The outcome of a good Rehabilitation Plan is the achievement of goals. It is also the satisfaction that comes with your regained sense of pride. Feel proud of yourself as you advance.
Participants attend the on-site program for approximately 3-4 hours several days each week. A variety of groups are offered to increase daily living skills. Transportation is available to and from the program and lunch is provided.
Participants are visited in their homes by rehabilitation counselors who support independent living in the home setting. In addition to in-home assisstance, the off-site program provides access to many community resources such as public transportation, shopping, banking and assisstance with appointments.
Using structured therapeutic approaches, our skilled professional counselors find ways to motivate and reintegrate program members. Individualized rehabilitation plans ensure a personal and effective outcome. Activities include assessments, rehab planning, skill building, and support networking. Groups include communication, socialization, self-awareness, mental health, medications, community resources, independent living skills, financial management, food preparation, hobbies, basic adult education, mobility training, and much more.
The PRP is an active program that uses community resources. Members enjoy a combination of inside and outside activities. There are frequent trips to museums, recreation centers (pool halls, bowling alleys, state parks, gymnasiums, movie/music/drama theaters, restaurants), and other interesting places and activities like libraries, shopping, sight-seeing, fishing, hiking, canoeing, biking, and dining out.
Our basic belief is that people learn in the real world, and that the rehabilitation process should maximize the use of real world situations. Overcoming the effects of mental illness is best attempted with interaction with lots of different types of people and in many settings. Our members will balance the learning of daily living skills at the program with the use and generalization of skills in the surrounding community. We use what we learn.
Case Management Services - Help with applications for HOUSING (supervised and independent, Section 8, CSA Grants, Shelter Plus Care), BENEFITS (SSI, SSDI, Medicaid, Medicare, Food Stamps, Cash Assistance, Pharmacy Assistance, Personal Adult Care (PAC)), JOBS, MEDICATIONS, TRANSPORTATION, APPOINTMENTS WITH PSYCHIATRISTS/COUNSELORS AND FAMILY RELATIONS.
Skills Assessment - The assessment helps to identify your strengths and weaknesses. We build on the strengths and improve the weaknesses.
Every person is unique and the assessment is like a snapshot of your skill level at the moment. Each skill is broken down into many levels to isolate the weaknesses and strengths.
Rehabilitation Plan - The Assessment provides us with a picture of your skills profile. The next step is developing the Rehabilitation Plan.
The Plan focuses on living skills that will help you gain independence. The skills that require improvement receive attention, and the Plan guides you and the Rehabilitation Counselor.
Specialized Learning - Using state-of-the-art rehabilitation techniques and a small counselor/client ratio yields personalized and effective skill learning. Small groups are the key to rapid and lasting social skill learning. The rehabilitation plan is presented daily to maintain therapeutic focus. You will become invested in getting well when you feel respected and involved with all aspects of the rehabilitation process.
